Children's Long-Term Program

The Children's Long-Term Support Home and Community-Based Medicaid Waivers (CLTS Waivers) provide a structure within which Medicaid funding is available to support children who are living at home or in the community and who have substantial limitations in multiple daily activities as a result of one or more of the following disabilities:

  • Developmental disabilities
  • Severe emotional disturbances
  • Physical disabilities

Each child and family will work with a support and service coordinator (SSC) that will help determine eligibility for the program.  Once that has been determined the SSC works with the family and child to develop their Individual Service Plan (ISP).  The CLTS Program uses a team approach to decision-making. It’s called Deciding Together.  The team includes the child, the family, SSC, and anyone else that the family wants to be a part of the team. This approach supports each family and child.  Funding can be used to support a range of different services that are identified based on an individual assessment of the child and his or her needs.
